The entrance is the mouth of the house. Facing (north / east / west / south) names the wall. Pada names the slot on that wall. Both are required.
Height, threshold and what sits behind the door
The main door should be the largest door, with a threshold. A toilet or stair immediately behind the entrance is a common apartment conflict — note it on the plan rather than guessing from the lobby.

North, North-East and East are traditionally favoured. The exact pada on that wall matters: a North door jammed into the North-West corner is not the same as Mukhya / Bhallat toward the centre-east of the North wall.
